The nightmare is finally over. Keep trying! It took me a week of farming Steel and Flame but it finally dropped eventually. Those were the only two I was missing, but had to bite to bullet and decided to do the duels instead after 2 weeks of grinding them. I promise they do! Iirc, nightmare is at 5%.
